I have a task set to trigger when a window opens. It sometimes works and sometimes doesn't. I created the trigger by dragging the icon over the open window, which it recognized. I saved the trigger and closed the properties. I reopened the window and the task didn't trigger. The Automate task service is running and the window is produced by radmin.exe.

any thoughts on how to force Automate to react to the open window?

Michael,
unfortunately window triggering in Automate is not reliable. I have the problem for years now, where everything works fine, then for an unknown reason it does no longer. It stops triggering either at all, or at least on some types of windows, not always the same ones.
I never found a correlation of this problem with anything.
The only way I know to escape the problem is stopping then restarting the Automate service.

N/A has been working for years upon my findings, in AM5, AM6, AM7/BPA7 and now AM8/BPA8, without any improvement in the situation.

The only possible workaround I could think of, since in your case it seems limited to one specific window, would be that instead of a triggered task, you have a permanently running task that waits for that window, and processes as necessary when the window pops up.
The waitforwindow action was never reported (so far, and as far as I know) as not responding to a window.
Or (better?) you could have that task triggered by the opening of the radmin.exe process, and beginning by a waitforwindow.
Process triggering is also known as reliable.

window triggering has not improved recently, neither in version 7 nor in version 8. The current version 9.0.0.25 is still suffering the same long lasting problems.
However my current beta testing of the upcoming version 9.0.1 has proven significantly more reliable in terms of window triggering.
I think it will be released by N/A in a matter of weeks, when most of its many early bugs are fixed.
