We had a situation where we inadvertently had two "Success" arrows between tasks. You could not see it in the designer because they were placed on top of each other. This caused two instances of the task to get executed. I don't think the designer should allow you to do this or at a minimum it should warn you when this happens.
We are using Automate BPA Server 9.0.1 |

I do apologize however this is by design.
